Electronic Medical Record system data is stored in a database such as Oracle or Microsoft SQL. Databases, while not accessible to the average user, can be manipulated, altered and data can be inserted into EMR systems and made to look like it was there all the time. Databases, however, track when information is altered or added to the database. Metadata ( data about data ) is generally created by the database system, outside of the EMR system. So manipulation of the EMR system may be detected by reviewing the raw data and the database logs
